To request services from Disability Connect Victoria click the button below to complete our a referral form, we will then give you a call to discuss your needs and to set up a meeting between you and our Intake Coordinator.
If you would like anyone else to be there, such as a family member, a support coordinator or anyone else in your support network we welcome and encourage them to join this meeting.
We'll have an initial meeting to fill in the ‘Client Intake’ form and sign DCV's service agreement.
Once both parties understand and agree to what services will be provided, we will ask for the service agreement to be signed so we can commence support.
Collecting the relevant assessments, reports and plans.
Depending on your personal circumstances this may include your NDIS Plan, assessments from Speech / Occupational therapists and/or health professionals, as well as any behaviour support and/or action plans
Start providing support
We will then use this information to work with our office team and ensure that our support workers are prepared to start providing support on your desired start date. Our office team will then set up an ongoing roster with a minimum team of two Support Workers to cover all shifts.
We will get you started with DCV services within 14 to 28 days from the moment we receive your referral.
